Waiver and Release
This is a release: Intending to be legally bound, I do hereby, for myself and my heirs, executor, administrators, and assignees, release and forever discharge all rights and claims which I may have or which may hereafter accrue to me against G5 Fitness LLC, coaches, and volunteers from the training or while participating.
